As beloved as our health services might be, they weren't designed to deal with the increase of long term conditions and so-called lifestyle diseases we see today, says Participal's Tara Hackett, Lead Guide and Project Developer for Wellogram - a new social care service in south London looking for a keen and confident individual to help take it to the next level.

Doctors and nurses face high caseloads and strict time limits, making it difficult to have the type of meaningful conversations with patients that lead to lasting change. We're trying to turn that around with Wellogram, a social health service that helps people feel in control of their habits and behaviour patterns.

Our guides spend 30 minutes per session with each patient, working together to determine what their goals are and strategies for reaching them. While we do measure numerical progress, the patient's own satisfaction with their health and wellbeing is our first priority. Helping people make small, sustainable changes has resulted in some great success stories.
